Hm... I think it depends on what you want to read. If you only want to read a story set in a certain universe or using similar topics, then it's sufficient if the writer knows the rough concept of a story. If you are more hoping for some kind of detailed crossover, of course it would be better if the writer knows the world in more detail...

Haha then I think my imagination went wild already XD In fact my brain went on to start plotting an Arashi-fic with Aiba as cute ghost already, inspired by your prompt and could totally imagine to write it for TOF... but it went into a completely other direction than you lined out somehow o.O (much more like Aimiya friendship with Aiba helping Nino to get together with Sho (yes, how did Sho end up there in that plot? XD Blame my OTP-braincells LOL)
I can already picture some of Arashi in similar roles like in "A Life"... brain, stop, I need to work on my Ninoexchange >.< Get to work there! Anyway, it's really an interesting AU. In my collab-fic with Wendy, Sho and Jun are doctors too and I really enjoyed these scenes.
